ABSTRACT
Introduction: Ovarian benign cyst are frequently managed by laparoscopy. Recently published studies advocate this aproach for well-selected patients with excessive size tumors. Case report: We present a case of a 21 years old female who presented in First Surgical Clinic Iasi with lower abdominal pain and bowel symptoms. The ultrasound examination showed a unilocular cyst measuring 23 by 14 cm, arising from the left ovary and occupying the entire pelvis and lower abdomen, with bilateral hydronephrosis. An open laparoscopy was performed. The cyst was drained with a laparoscopic puncture needle introduced through the right accessory trocar sleeve and connected to a surgical suction system. Aspiration was followed by laparoscopic adnexectomy. Histopathological exam revealed the presence of a mucinous cystadenoma. Postoperative recovery was uneventful and was discharged 3 days after operation. Conclusion: Laparoscopic drainage of ovarian cyst followed by adnexectomy is feasible and may be indicated for large ovarian cyst after careful preoperative evaluation.
