Parkinsonism and basal ganglion involvement due to antiemeticdrugs, such as metoclopramide, is rarely encountered.However, bilateral basal ganglia and brain steminvolvement has not yet been reported in patients withdrug-induced Parkinsonism.A 52-year-old male patient with laryngeal carcinoma wastreated with docetaxel, cisplatin, and 5-fluorouracil (DCF)chemotherapy. Because of severe nausea and vomiting15 days after the first course of chemotherapy, metoclopramidewas administered. Parkinsonism symptomswere observed on the 6th day of metoclopramide therapy.Cerebral MRI revealed bilateral basal ganglia and brainstem involvement. Metoclopramide therapy was promptlydiscontinued and oral biperiden was commenced. Parkinsonismfindings almost completely improved on hiscontrol examination performed after one month. He wasnot given metoclopramide during further chemotherapycourses. Cerebral MRI taken after three months revealedthat the lesions have completely relieved.This is an extremely rare case of metoclopramide-inducedparkinsonism together with brain stem and basal ganglioninvolvement in a patient with laryngeal carcinoma treatedby chemotherapy. The clinical and radiological improvementwas observed with the cessation of the causativedrug and biperiden therapy.
