Regarding the increasing prevalence and mortality of asthma in the past 2 decades and recent achievements in the past 10 year which indicated that asthma is a chronic inflammatory disorder, asthma therapy has changed dramatically with increased emphasis on the use of anti-inflammatory medications. Environmental allergens and/or irritants cause inflammatory reactions in lung tissue and trigger recurrent episodes of cough, dyspnea, wheezing and variable airflow limitation and increased airway responsiveness. Thus the most effective management is to prevent this inflammatory process by eliminating the causal factors through educating the patients to develop a partnership in monitoring the severity of the disease, avoid or control triggers, establish plans for long term management, and managing exacerbations and provide regular follow up care. The recommendations in this article link the rationale for the therapies to the scientific understanding of asthma and are based on controlled clinical studies, clinical experience and literature review.
