ABSTRACT
A patient with morbid obesity and with a thirteen-year-history of a metabolic syndrome, fully documented through clinical and laboratory tests, yet without the manifestation of diabetes was reported. The patient had overcome a deep venous thrombosis of the anterior tibial vein and was suffering from recidivous phlebitis of the lesser saphenous vein. The patient’s thrombophilic status and the clinical picture of decelerated healing of the leg ulcer can be explained: a) by the presence of prothrombin mutation gene, b) by the presence of unambiguous latent inflammation activity, which is part of the clinical picture of morbid obesity and the patient’s metabolic syndrom and which leads to the activation of platelets, c) by pathological concentrations of adipocytokines (adiponectin, resistin, leptin). Surgical treatment can be performed only after a radical reduction of the patient’s weight. Anticoagulatory therapy with warfarin have a positive therapeutic effect.
