Ocena funkcji intelektualnych pacjentów z chorobą afektywną dwubiegunową w okresie remisji

The aim of the study was to assess the intellectualfunctioning of patients with bipolar disorder in remissionand to delineate putative cognitive deficits in verbal andnonverbal functions and their associations with thecourse of illness.Material and method: Fifteen patients with bipolardisorder (BD) in remission lasting for at least 4 months(4 male, 11 female; mean age 53.1±11.0 years;education level 13.0±3.2 years) and 15 age-, genderandeducation-matched healthy controls were included.Mental state was assessed using the HamiltonDepression Rating Scale (HAMD), and Young ManiaScale (YMS). Mini Mental State Examination (dementiascreening) and Wechsler Adult IntelligenceScale-Revised (WAIS-R) (assessment of intelligencequotient) were administered.Results: Bipolar patients scored significantly lower thancontrols in full scale and Performance IQ (PIQ) ofWechsler Adult Intelligence Scale-Revised but notverbal measures of IQ. BD patients had significantlylower scores than controls in several performancesubtests: Digit Symbols, Block Design, and PictureCompletion suggesting visuospatial and perceptualorganization dysfunctions. Bipolar patients also scoredsignificantly worse on one verbal subtest. Similarities,which may be associated with abstract thinkingdifficulties in the patients group. Cognitive deficits wereassociated with the course of affective disorder includingnumber of affective episodes, especially depressions.Conclusions: Worse results in performance IQ inbipolar patients may suggest lateralized dysfunction ofthe right hemisphere (RH) which plays a significant rolein visuospatial processing and perceptual organizationof intellectual functioning.
