THERAPEUTIC APPROACH IN CRITICAL LIMB ISCHEMIA

ABSTRACT
Critical limb ischemia is the most severe manifestation of peripheral arterial disease. The gold of this study is to assess the prevalence of risk factor, associated comorbidities and therapeutic approach in a group of patients hospitalized in the Medical Clinic with critical limb ischemia. Material and methods: During 2005 – 2010, were evaluated 1301 patients diagnosed with critical limb ischemia performing ankle-brachial index and arteriography. Results: I evaluated the treatment received by patients: 100% aggressive therapy to modify atherosclerotic risk factors, 99% antiplatelet therapy, 100% vasodilator treatment, 94% administration of analgesics, 12,75% treatment of infection, 1,2% thrombolytic therapy, 55% surgical bypass, 7% percutaneous transluminal angioplasties, 6% lumbar sympathectomy, 32% risk of amputation. Conclusions: There is a tardive diagnosis for peripheral arterial disease, related with advanced clinical status and fewer opportunities for treatment. It is strongly necessary to create an integrated system for prevention, active detection and aggressive therapy for peripheral arterial disease.
