A tool for the analysis of DRSs and surgical activity in hospitals in the Ile-de-France

ABSTRACT
In order to more clearly delineate the surgical component in the second generation regional project for the organization of health care in the Ile de France, we performed a descriptive study using datas taken from the program for the medicalization of information systems (PMSI) during the second semester of 1997, aimed at quantifying the overall surgical activity in the Ile-de-France in view of helping experts formulate future recommendations. Only hospital stays classified as surgical in the diagnostic- related groups (GHM) were used. A description of each activity according to specialty and the relative weight in resources deployed was established with the help of the « Tool for PMSI Analysis » (OAP), a new method for grouping the GHM developed by the regional technical committee for medical information (COTRIM) of the Ile-de- France. The main results of this study underscore the relative importance of private, for-profit clinics in the production of surgical hospital stays, particularly in the ambulatory setting, as well as the significant role played by the Assistance Publique-Hôpitaux de Paris in the care of both young children and patients requiring highly specialized medical care. By offering a synthetic view of health-care facilities according to specific activities, OAP appears to be a useful tool for exploiting datas contained in the PMSI and for future planning in the regions of France.
