Xanthogranulomatous pyelonephritis

A recently diagnosed case of xanthogranulomatous pyelonephritis in a 5.5-year-old boy is reported who was referred to us because of mild fever, loss of appetite and abdominal distension lasting for 3 months. Urinalysis showed mild proteinuria, with 10-15 leucocytes, 3-4 erythrocytes and 15-20 granular cast per microscopic field. An abdominal mass extending from right costovertebral angel down to the right lower quadrant was palpable. Intravenous pyelography, sonography and ct scan showed hydronephrosis of the non-filtrating right kidney with occlusive nephrolithiasis. Nephrectomy and appendectomy was preformed because of a retroperitoneal hard mass extending from right renal region to the pelvis with adhesions to the surrounding tissues that contained green colored pus. Klebsiella and proteus were isolated from the pus. Pathology of the kidney showed xanthogranulomatous pyelonephritis
